Exosome Order form

If a provider choses to purchase exosomes, I understand there is the online order form via our portal. In the training course it recommends having the provider fill it out themselves to avoid any mistakes. What if I am doing a remote call and they chose to purchase, is there a link or form I can send them to fill out?
