Nope. Even if they have employees, as long as it is a sole proprietorship and there are no other providers operating under the entity. If they incorporate, even as a "sole owner" of the organization, they will need a group NPI. As a disclaimer: I am not a lawyer or even qualified to give such advice, however, I am a provider and would never consider NOT incorporating due to the protections it offers from personal liabilities in the litigious environment we live in.
